The Improve Care Now team continues to innovate and lead towards its mission of improving care and health of children and adolescents with ulcerative colitis and Crohn’s Disease. Here are a few of the many activities the team is currently working on:
- Bring more patients, families and doctors to participate in Improve Care Now. Already, patients in the program are experiencing success in getting their disease into remission and keeping it there. One of the fastest ways to help more patients to do the same is to simply get them, their doctors and their medical facilities into the program.
- Continuing our work to improve care and treatment. The members of the Improve Care Now team continue to find and share new ways to help patients do better. As part of our efforts, we have been developing and making research, tools, techniques and systems for care improvement available to all. Doctors and patients continue to increase their use of these capabilities, further improving health of patients.
- Accelerate our efforts to improve patient health. Improving patient health means:
- Increasing the percentage and number of patients who are in remission (patients feel well, function normally, have no symptoms of disease);
- Reducing the frequency of relapses (reduce how often a patient who is well develops symptoms again);
- Reducing the frequency of hospitalizations and surgery; and
- Increasing the percentage and number of patients with sustained remission (being in remission for 12 months or more).
- Helping patients and families to take control and better manage their own care. The best treatments in the world are not effective if patients can't or don’t use them. Improve Care Now is developing and offering free tools and planners to help patients and their families to make sure that they are receiving the best care available.
