Improve Care Now provides powerful tools and reports to your doctors, enabling them to monitor health and disease status of all patients, medications, medication doses, serious side effects, regular visits and to identify and provide extra care for patients needing more help.
Complete data collected at each visit
Your doctor will be asking the necessary questions, making the necessary measurements and tracking all of the results in the Improve Care Now system in order to build the best picture of your overall health and progress.
Assessment of nutrition and growth
Your doctor will be watching nutrition and growth closely, and will be able to make changes to receive the best possible nutrition and health.
Assessment of disease severity
Your doctor will formally assess the disease activity and your health at each visit, to track changes over time and make changes when needed.
