Here’s How to Use Health Tracker

 1 After selecting the online Health Tracker for Ulcerative Colitis or Health Tracker for Crohn's Disease, answer a few questions about how you are feeling today, and enter your age and your email address (if you want a copy of your Health Tracker emailed to you).

 2 Health Tracker will calculate and display your Health Score.

 3 Come back regularly to use Health Tracker to see how you are feeling over time. You’ll be able to see trends that may help you to better manage your own care and health.  And by printing the information you enter and your Health Score each time, you’ll be creating a very valuable record that you can take to your doctor visits.  This will help your doctor to see how you are doing over time, and may help them to help you feel better and stay well!

Important: Your Health Score is NOT an indication of how healthy you are, and does not indicate either that you are doing well or not so well.  Health Tracker is NOT a substitute for going to the doctor, and you should continue to do that on a regular basis.  Instead, Health Tracker simply allows you to keep track of how you are feeling and to share that information with your doctors if you wish.

About the Information Collected by Health Tracker

All of the information that you enter is for you and you alone. Health Tracker does not collect any information that can be used to identify you. Once you click Finished or Cancel at the end of using Health Tracker, all information you entered is discarded.  However, you may choose to keep the data that you enter and your Health Score by two methods, if you’d like:

• You may print the information, and keep the paper copy. We recommend that you do this regularly, and especially that you share this information with your doctor, but it’s up to you.
• You may email the information to yourself or to someone else. Please remember that your information is confidential, and it’s up to you to decide whether to email the information to somebody, and to make sure that you enter the correct email address.

Other Security Tips

These tips are especially important if you are working on somebody else’s computer, or a computer in a public place such as a library or school!

• If you print your information, be sure to take your printed copy with you and store it in a safe place. If you are printing on a public computer or printer, please make sure that you know where the printer is so you can get your information as soon as it’s printed.
• If you email the information to anyone, please be careful to enter the correct email address. If you are not sure of the correct email address, then please print your Health Tracker and keep it in a safe place.
• Be sure to click the Finished button or the Cancel button when you are all done. This erases all of your information so that others cannot see it.

How the Information You Enter Is Used

Information that you enter to Health Tracker is used only to:

• Calculate your Health Score.
• Print the information you enter and your Health Score, if you choose to.
• Email the information you enter and your Health Score, if you choose to.

This information is not transmitted, printed or otherwise shared except as described above.  When you click the Cancel or Finished button, the information that you entered is discarded.
