The Visit Planner is part of the ImproveCareNow Model Care Guideline. This guideline has been developed by doctors participating in the ImproveCareNow collaborative, and is based on the "chronic illness care model." The chronic illness care model has been developed based on many years of research and has been successful in helping patients to manage chronic illnesses.
Here's How to Use the Visit Planner
1 Before your next doctor visit, select the Visit Planner for Ulcerative Colitis or the Visit Planner for Crohn's Disease.
2 Answer a few questions about how you or your child has been feeling. You may optionally enter your email address if you'd like your answers to be emailed to you for your records.
3 Once you submit your answers, you'll be presented with a printable page displaying the Visit Planner questions and your answers. Print this page, and take it to your next doctor visit. By sharing this important information with your doctor, you'll be an active participant in helping your doctor to help you or your child feel better. As this tool will continue to be enhanced as new techniques are learned, be sure to come back to this website to complete a Visit Planner before every visit to your doctor.

No Information Is Collected by Visit Planner
All of the information that you enter is for you and you alone. Visit Planner does not collect any information that can be used to identify you. Once you click Finished or Cancel at the end of using Visit Planner, all information you entered is discarded. However, you may choose to keep the data that you enter by two methods, if you’d like:
- You may print the information, and keep the paper copy. We recommend that you do this regularly, and especially that you share this information with your doctor, but it’s up to you.
- You may email the information to yourself or to someone else. Please remember that your information is confidential, and it’s up to you to decide whether to email the information to somebody, and to make sure that you enter the correct email address.
Suggested Security Tips
These tips are especially important if you are working on somebody else’s computer, or a computer in a public place such as a library or school!
- If you print your information, be sure to take your printed copy with you and store it in a safe place. If you are printing on a public computer or printer, please make sure that you know where the printer is so you can get your information as soon as it’s printed.
- If you email the information to anyone, please be careful to enter the correct email address. If you are not sure of the correct email address, then please print your Visit Planner and keep it in a safe place.
- Be sure to click the Finished button or the Cancel button when you are all done. This erases all of your information so that others cannot see it.
How the Information You Enter Is Used
Information that you enter to Visit Planner is used only to:
- Print the information you enter, if you choose to.
- Email the information you enter, if you choose to.
This information is not transmitted, printed or otherwise shared except as described above. When you click the Cancel or Finished button, the information that you entered is discarded.
