If your care center is in the Foundations Learning Lab
- Download a copy of the “ICN Roles and Responsibilities” file and complete it with your ICN team. Email the completed template (or your own format) to [email protected] by Wednesday, August 28
- Download the file by selecting the 3 dots to the right of the file name and selecting “download file”.
- If you already have ICN roles and responsibilities documented for your team, you can use and submit your current format.
- Complete this survey to assess your center’s performance in relation to the Pathway to Mastery advancement criteria. Additional instructions for where to find certain measures can be found on the Fall 2024 Prework_PtM Criteria document.
- Even if you are unable to attend the conference, we are asking all Foundations centers to complete the pre-work.
If your care center is in the Pre-Visit Planning and Population Management Learning Lab

As pre-work for the upcoming ICN Community Conference, please work with your team to complete this Center Self-Assessment by COB Wednesday, September 4, 2024. This tool will be used to guide your team in assessing center-level outcomes, QI skills, Project Management, and ICN network activities typically needed to conduct reliable Pre-Visit Planning and Population Management. We will use this information to tailor content at the Community Conference and in 2025 Learning Labs to best meet the needs of your teams.

Once you’ve completed your Self-Assessment, don’t forget to complete the network-wide ICN Community Conference Pre-Work. Be sure to have the Outcomes Registry pulled up to help you complete this!
If your care center is in the Clinical Standardization/Personalized Care Learning Lab
Please submit the CS/PC Fall CC Pre-Work Survey by Wednesday, August 28. There will be an opportunity to discuss during the CSPC August 20 Learning Lab.
The Survey will take approximately 60 minutes to complete. To ensure comprehensive and accurate responses, please coordinate with your ICN physician leader and an ICN coordinator to complete the survey.
Please come to the August 20 Learning Lab prepared to ask any questions you might have. We also advise you to schedule time with your physician leader to work through the survey questions together after the Learning Lab webinar.
The survey consists of three parts.
Part 1. PtM LL Criteria Assessment:
This is a network initiative. As preparations begin for the 2024 Fall Community Conference, we ask teams to take a moment and assess your center's performance in relation to the PtM advancement criteria. Aggregate results will be shared during the PtM Advancement Celebration and in the CS/PC LL Breakout session.
Part 2. Identifying High Impact Implementation Factors
Think about your care center's experience so far implementing the therapeutic drug monitoring (TDM) process described in the Adalimumab and Infliximab treat-to-target algorithms. Look over the implementation factors listed in Column A of the table, and indicate their expected impact on the implementation of TDM in your center over the next 6-9 months (high impact, low impact, or no impact). Focus on the factors that are most important to you right now. If your care center has other relevant implementation factors that are not listed, please add them in the text box at the end of the table.
Part 3. Mapping Change Ideas to High Impact Implementation Factors
Your care center's success in implementing TDM will improve if you use strategies that target the high impact implementation factors you've identified. Review this Excel sheet with a list of 26 specific change ideas for TDM (Tab 1) and the top 3 change ideas previously selected by each CS/PC care center (Tab 2). If your care center does not have change ideas in Tab 2, go directly to Step 2. Otherwise, do Step 1 first.
- If your care center has change ideas in Tab 2, indicate which high impact implementation factors you identified in Part 2 (Column A) could be addressed by these change ideas, using the drop-down menu in Column B. More than one change idea can target each factor, and one change idea can address multiple factors. If a change idea doesn't map to any of the factors, don't try to force a match. When you are done with all the high impact factors, go to Step 2.
- Review the change ideas in Tab 1. If you would like to consider any of these ideas for addressing any of your high impact implementation factors, add them to Column B. If you already have change ideas from Step 1, add any new ones you would like to consider. If you do not, select the ones you would like to consider from the list. You have up to 4 choices for each high impact factor, but you do not have to pick 4 for each. Try to have at least one change idea mapped to each high impact factor.
