After the Fall 2025 Community Conference, we will share the Conference Evaluation with all attendees. If you are seeking CME, CNE, IPCE, or MOC Part II credit, please see the steps below. If you have any questions or concerns, please email [email protected].
Claiming Continuing Education Credit
Requirements for claiming CME/CNE/IPCE Credit:
- Register and attend the Fall 2025 Community Conference
- Attend the Community Conference
- Submit your Attestation Form to the CCHMC Cloud
⭐ Your Attestation Form must be submitted by 10/19/2025 to receive any form of credit ⭐
CME/CNE/IPCE Credit Hours:
Please review the Education Handout if you are planning to claim education credit. The following credits can be awarded:
- 10.75 credit hours can be awarded for AMA PRA Category 1 Credit(s)™
- 10.75 credit hours for Continuing Nursing Education (CNE) contact hours
- 10.75 credits for Interprofessional Continuing Education (IPCE) credits
- 6.00 Social Workers CEU points
Social Worker CEU Credit
Requirements for Earning up to 6 CEU credits:
- Attend the approved sessions eligible for CEU credit at the Community Conference. The approved sessions are denoted with 💙 on the agenda.
- Complete the sign-in sheet and select the sessions that you attended
- Complete the general conference evaluation and Attestation Form on the CCHMC Cloud
- Complete the unique session evaluation and provide responses for sessions that are attended by September 26, 2025 via a digital survey that will be emailed to you from Linda Demosthenes (CCHMC) after the event.
