IGNITE - A Parent's Perspective on Healthcare Learning Networks

Ignite Talks are a true highlight of ImproveCareNow Community Conferences - whether we are meeting in-person or live online. This spring, at our Live Online Community Conference, we were honored to listen as Jane Weyer shared her personal story as parent to a child diagnosed not only with Crohn's disease, but also Primary Sclerosing Cholangitis (PSC) - an autoimmune liver condition affecting around 30,000 people in the US. Jane, and her daughter Nicole, didn't have the information or resources they wanted at the beginning of their PSC journey. So, they rolled up their sleeves, leaned on the strong foundation of connection and improvement built by the ImproveCareNow Learning Health Network, and together with other visionaries founded A-LiNK (the Autoimmune Liver Disease Network for Kids). A-LiNK Connections, the caregiver/patient community, was established to collaborate, advocate, support, and educate families on this journey with them.

There is no better outlet for understanding your diseases than through other patients

My name is Nicole and I am 21 years old. I was diagnosed with Crohn's disease on my 16th birthday and Primary Sclerosing Cholangitis (PSC) just a few months later. These diagnoses were heartbreaking, but I did not let them stop me. I am graduating magna cum laude with a Bachelor's of Science in Biology and will be returning to my hometown of Cincinnati, Ohio to work as a Genetic Counseling Assistant to pursue my passion in genetics.


Creating an ICN-like learning network for autoimmune liver disease

Hi, I’m Jane, “Mom” to beautiful 20-year-old Nicole. I serve as a parent representative at CCHMC for ImproveCareNow (ICN) and our local network called: IBDevoted. I am also a Board member of the local chapter of the Crohn’s & Colitis Foundation. In my professional career, I work for a large Catholic healthcare system handling medical malpractice cases. While I’m not technically a healthcare professional, I joke that “I play one on TV”!
