ImproveCareNow Community Conferences happen twice a year. While our community works together across the miles throughout the year, these in-person meetings provide an opportunity to work on developing and spreading QI and research skills and approaches; fueling our Network's use of data to drive and evaluate QI efforts; and developing partnerships that will lead to improved care processes and outcomes in pediatric IBD. Join us as we continue to co-develop our three pathways to work towards our common missionto transform the health, care and costs for all children and adolescents with Inflammatory Bowel Disease.

September 19-21 we will welcome more than 200 patients, parents, clinicians, researchers, improvers, collaborators, and friends back to Chicago, IL for our Fall 2019 Community Conference. Take a look at our conference agenda ⬇⬇

In ImproveCareNow we have a motto: all teach, all learn, which we really take to heart. As you review our agenda, notice that nine sessions have been developed by members of our community, including patients and parents (hint: look for the tiny ICN logo). These sessions represent the interests and experiences of our diverse community, and a commitment to sharing seamlessly so others in ImproveCareNow can steal shamelessly and more people living with IBD can benefit.

At this Community Conference will continue exploring the next evolution of our Learning Healthcare System - our Pathway to Mastery. By meeting participating centers where they are in their ICN journey - from the foundational work of identifying and enrolling all eligible IBD patients, to implementing changes that lead to improved remission rates, and ultimately developing reliable systems for sustaining remission once it's been achieved - we are developing and refining a new framework that will provide our centers with content and support that is tailored to their individual performance, strengths and opportunities.

In addition to focusing on our Three Pathways, and how they help us successfully achieve our mission, all Community Conference attendees will spend time working together to share and develop new and innovative approaches to improving care, furthering research, and building community to catalyze improvements in the health, care & costs of young patients with Crohn’s disease and ulcerative colitis. Together we will renew our commitment to co-producing care that is timely, proactive and reliable – so that our young people all have the very best chance at living healthy lives doing the things they love.

As always, we strive to offer something for everybody at our Community Conferences!

We will once again be offering Part 2 Maintenance of Certification credit for pediatric gastroenterologists, as well as CME/CNE credits for medical professionals attending the Community Conference.
