Dear Improvers - 

We are thrilled you will be joining us at the ImproveCareNow Fall 2019 Community Conference in Chicago on September 19-21, 2019, where we will share and develop new and innovative approaches to improve care, further research, and build community. You can review the draft agenda here. As always, we will share the final agenda and breakout selections once CME/CNE credit is finalized.

To support clinicians in making the case for local funding to participate in the Community Conference, we have created a conference justification template letter. Please consider customizing the template, should you need to request funding from senior leaders at your care center.

What Do We Want To Accomplish?

As with past ImproveCareNow Community Conferences, we have planned something for everyone, regardless of your role, current level of quality improvement knowledge, or the current focus of your research or improvement efforts. Our Community Conference objectives are for attendees to have:

  • Participated in their new Learning Labs and have assessed how to integrate the associated interventions into their care center workflows.
  • Acquired new examples and ideas on how to partner with parents and patients in meaningful ways to grow their local improvement community.
  • Planned to test new changes at their center in alignment with 90-day goals and Learning Labs.
  • Demonstrated enhanced ImproveCareNow quality improvement, research, and community engagement skills.

Next Steps:

As you prepare for the Fall 2019 Community Conference, please do the following:

  • Register and make your room reservations.
  • Review and complete your pre-work. For this conference, pre-work will be determined and communicated by each Learning Lab. Please look for communication from your Learning Lab QIC.
  • Bookmark this page on the ICN website. As in previous conferences, we will be largely paperless. You will be able to access the conference agenda, presentations, and handouts on the ICN Website. You can access this link on your phone and tablet.

An Important Note About Attendance:

We will adjourn on Saturday, September 21 at 12PM Central Time. We recognize and appreciate that you are sharing your time with us and ending the conference at noon should allow you to return home earlier in the day. Therefore, we strongly urge you to schedule your flight home to allow you to stay until the very end. We are planning an engaging program that requires team participation until noon.

Pre-work for the Fall 2019 Community Conference is determined by your center's Learning Lab.


Fundamentals - Due Sept. 13

Submit the Fundamentals Center Assessment to Kim Shelly, and bring a copy of your completed Center Assessment and QI Dashboard to the Conference.


Reinforcing Foundations - Due Sept. 13

Submit the Reinforcing Foundations Center Assessment to Theresa Todd.


Multidisciplinary PVP - Due Sept. 13

Submit the following to Mary Bauer:

  • A blank copy of your current PVP form.
  • Responses to these questions:
    • What improvements need to be made to the existing ICN PVP form in the ICN2 Registry to make it an effective tool?
    • What are the key components of an ideal PVP form?

Population Management - Due Sept. 13

Track failures that occur during your Population Management Process using this document and submit the final tallies via this survey.


Adherence - Bring printed or electronic copy to the conference

Select 1-2 weeks of patient appointments to review (can be prospective or retrospective) and answer the questions associated with each process step on this document.


Clinical Standardization/Personalized Care - Due Sept. 17

Analyze you center's patient population (outlined in the survey) and submit final tallies via this survey.

Please refer to the Breakout Descriptions document as you decide which breakout sessions to attend at the Fall 2019 ImproveCareNow Community Conference. Please consult with colleagues attending from your center (including parent and patient partners) to maximize your team’s exposure to these sessions—plan to divide and conquer!
