ImproveCareNow is pleased and proud to introduce an excellent new self-management resource for young patients and their families who are living with Crohn’s Disease and ulcerative colitis (Inflammatory Bowel Disease or IBD). Living Well with Inflammatory Bowel Disease: A Self-Management Handbook for Patients & Families with IBD, or “the Handbook”, is a comprehensive resource for patients and families who are learning about these conditions.  

It is also a companion and guide for those who are living with IBD and want to participate more actively in their own care. The Handbook is the result of collaboration between pediatric IBD experts at Nationwide Children’s Hospital (Columbus, OH) and Cincinnati Children’s Hospital Medical Center (Cincinnati, OH).  It combines comprehensive clinical knowledge with practical tips and suggestions from parents and kids who are living with IBD.

Clinicians and supporters of the ImproveCareNow Network have joined together and are dedicated to improving the care and lives of kids with IBD.  Empowering all patients and their families to interact more effectively with their health care teams is one aim of the Network's top aims.  ImproveCareNow knows that when cooperation develops between patients, families and health care teams, the results can be incredibly positive. 

The Handbook has been highlighted by the Collaborative Chronic Care Network (C3N) in their monthly newsletter, the HIVE (August 2011 edition).  ImproveCareNow and C3N have teamed up, and together are working to change the face of chronic illness care (www.c3nproject.org).
