ImproveCareNow Community Conferences happen twice a year. While our community works together across the miles throughout the year, these in-person conferences provide an opportunity to work on developing and spreading QI and research skills and approaches; fueling our Network's use of data to drive and evaluate QI efforts; and developing partnerships that will lead to improved care processes and outcomes in pediatric IBD.
September 13-15 we will welcome patients, parents, clinicians, researchers, improvers, collaborators, and friends to the Fall 2018 Community Conference in Chicago, Illinois and we hope you’ll join us too.

Take a look at our conference agenda! In ImproveCareNow we have a motto: all teach, all learn, which we really take to heart. As you review our agenda, notice those sessions that have been developed by members of our community, including patients and parents (hint: look for the tiny ICN logo). These sessions represent the interests and experiences of diverse community members, and a commitment to share seamlessly so others in ImproveCareNow can steal shamelessly and more patients with IBD can benefit.
Patients and parents are not only attendees at our Community Conference, they are presenters, facilitators, co-designers and active participants. Here are some sessions they're directly involved in:
- Best Practices for Identifying Parent and Patient Leaders at Your Care Center
- Development and Testing of the Transfer Toolkit – Lessons Learned and Changes to Test at Your Center
- Digital Tools to Support Your Team, Virtual Meetings, Online Organizational Tools, and Communications Platforms
- Managing Suicidal Ideation in the Care of Kids with IBD, Skills Training for Providers, Parents, Patients and Others
- Early Engagement Campaign Results from Two ImproveCareNow Centers – Successful Ideas You Can Apply at Home
- Quality of Life with IBD, A Guide to Supporting Your Patients with Accommodations
Additionally, all members of the community (including clinicians, coordinators, parents, and patients) will be working together actively to develop leadership and a culture that will allow us to continue to catalyze improvement in the health, care & costs of young patients with Crohn’s disease and ulcerative colitis. Together we will renew our commitment to co-producing care that is timely, proactive and reliable – so that every child has the very best chance at living a healthy life doing the things they love.
As always, we strive to offer something for everybody at our Community Conferences!
- Pediatric gastroenterologists and clinical care team members, joining us at the Community Conference, can take advantage of available Part 2 Maintenance of Certification (MOC) credit and CME/CNE credits.
- Learning tracks organize our diverse teaching and learning opportunities, making it easy to focus your conference experience in a single track, or diversify with a blend of breakouts from different tracks. Tracks include: QI, Clinical, Research, Data, Community Building & Engagement, and Other.
- Dedicated team time, role-specific breakouts and learning lab collaboration sessions offer protected time for focused discussion, brainstorming and planning of how to bring Community Conference learnings back to local centers. Teams learn and share how to spread important innovations more broadly across the ImproveCareNow Network so more teams & families can benefit.
- And, of course, there is always "Happy Trails" 🎵

Our Fall 2018 Community Conference hashtag - #ICNCC18F - was created via Symplur and the Healthcare Hashtag Project, which is aimed at connecting the dots in healthcare social media. By including #ICNCC18F in posts and tweets from and about the Community Conference we can document and share learning and ideas, make connections with our colleagues and community, and spread awareness of ImproveCareNow and IBD.
