ImproveCareNow aims to transform the health, care and costs for all children and adolescents with Crohn’s disease and ulcerative colitis by building a sustainable collaborative chronic care network. ImproveCareNow enables patients, families, clinicians and researchers to work together in a learning health care system to accelerate innovation, discovery and the application of new knowledge.

OUR RESULTS
ImproveCareNow has raised the proportion of children and youth with Crohn’s disease and ulcerative colitis who are in remission from 55% to 82% since 2007, without new medications. ImproveCareNow also has the largest and fastest growing pediatric IBD registry worldwide, called ICN2. See more of Our Success.
